Injectables do not apply to the membership program.
Fees may be applied toward the costs associated with injectable and filler services only (e.g., Dysport, Restylane, PRP injections, and etc).

6 Month Agreement – automatic payments will continue after 6 Month agreement unless guest wants to cancel.
10 Days’ notice before next bill day to avoid additional charges if guest wishes to cancel. Must come in person to fill out cancellation form.
If guest wishes to break agreement, doctors’ note is required stating they are unable to receive treatment.
Unused treatments will roll over, upon cancelling membership AFTER 6 Month agreement they will have 30 days to use any rolled over services.
If they are breaking the agreement, the unused services will be terminated.
***Eligibility for monthly TANZANITE body contouring program is excluded from the body contouring monthly maintenance service program but can be part of the initial treatment plan. TANZANITE members can purchase additional monthly services for a fee equal to their monthly membership fee.
